An active Texas physical therapist assistant license is required.
Home health experience is preferred.
Strong communication and patient care skills are needed.
Ability to work independently in a home setting is required.
Reliable transportation and a valid driver's license are required.
Kinnser/WellSky electronic medical record experience is preferred.

Responsibilities

Provide physical therapy treatments under the direction of a Physical Therapist.
Assist patients with exercises to improve strength, mobility, and balance.
Monitor patient progress and report changes to the supervising Physical Therapist.
Educate patients and caregivers on home exercise programs and safety.
Document visits accurately and in a timely manner.
Support patient goals and functional independence.

Home health experience.
Kinnser/WellSky electronic medical record experience.
