Requirements
  • A bachelor's degree in Social Work, Psychology, Education, Rehabilitation, Recreation Therapy, Sociology, Nursing, or a related field approved by the Illinois Department of Human Services, with an alternate qualification for certain candidates of at least five years of developmental-disability experience and a high school diploma.
  • At least one year of experience working with people with developmental disabilities.
  • At least one year of supervisory experience.
  • Successfully complete company-provided CPR, first aid, Crisis Prevention Institute, direct support professional, and medication administration training within the first four months and annually thereafter.
  • Obtain an Illinois Food Service and Sanitation certificate within the first six months and renew it as required.
  • Pass the state-required CASAS literacy assessment before employment.
  • Be proficient with Microsoft Word, Excel, and Outlook.
  • Pass criminal background, state and federal registry, DCFS Abuse and Neglect Tracking System, and driving-history checks.
  • Maintain a valid driver's license and meet the applicable age requirements to drive a company van.
  • Use a personally insured vehicle for company-related travel.
Responsibilities
  • Promote a person-centered approach that supports resident choice, independence, and community inclusion.
  • Assist residents in developing daily living, communication, social, and self-advocacy skills and ensure services align with individualized plans.
  • Monitor resident progress and outcomes and recommend program modifications as needed.
  • Complete and oversee required documentation accurately, timely, and in compliance with regulatory standards.
  • Develop, implement, and maintain Individual Habilitation Plans, Individual Service Plans, Individual Active Treatment Plans, assessments, and reports.
  • Oversee resident finances and personal funds according to agency policies and interdisciplinary team recommendations.
  • Foster relationships with residents, families, guardians, case managers, and community partners.
  • Plan, coordinate, and supervise service delivery and participate in admissions, screenings, assessments, and placement decisions.
  • Serve as chairperson of the Interdisciplinary Team and facilitate meetings with guardians, Independent Service Coordination agencies, healthcare providers, and other stakeholders.
  • Ensure treatment plans are developed, implemented, reviewed, and revised according to active treatment principles and regulatory requirements.
  • Coordinate medical, dental, behavioral health, and specialized services.
  • Monitor resident health and well-being and respond to medical concerns, emergencies, and changes in condition.
  • Support family and guardian involvement in resident care and program planning.
  • Maintain a safe, clean, and welcoming residential environment.
  • Enforce house rules, program regulations, agency policies, and resident rights.
  • Conduct safety inspections and ensure compliance with Occupational Safety and Health Administration standards and company safety procedures.
  • Ensure appropriate use of personal protective equipment and infection-control practices.
  • Investigate, document, and report accidents, injuries, incidents, illnesses, and unusual occurrences.
  • Participate in Human Rights and Behavior Management Committee activities.
  • Coordinate maintenance, repairs, and upkeep of the residence, grounds, furnishings, and equipment.
  • Report significant maintenance concerns and capital-improvement needs to administration.
  • Manage site expenditures, approve purchases, maintain inventory controls, and monitor supply levels.
  • Submit receipts, invoices, purchase orders, and financial documentation according to established procedures.
  • Assist with budget planning, financial reporting, expense monitoring, capital-improvement recommendations, and operational enhancements.
  • Support administrative initiatives, audits, licensing reviews, accreditation surveys, and funding-source inspections.
  • Assist with community relations and represent the organization during meetings, site visits, and public events.
  • Ensure staff training on policies, procedures, resident support plans, medication administration, safety practices, and regulatory requirements.
  • Provide leadership and direction for assigned residential staff.
  • Monitor staffing levels and schedules to ensure adequate coverage and quality service delivery.
  • Recruit, interview, train, schedule, supervise, coach, and evaluate approximately 8–12 employees.
  • Address employee concerns, performance issues, and disciplinary matters with Human Resources and agency leadership.
  • Conduct performance evaluations and provide feedback, mentoring, and professional-development opportunities.
  • Promote employee engagement and maintain a positive work environment aligned with organizational goals.
