Requirements
  • Requires bachelor’s degree in psychology, Social Work, or directly related field
  • 6 months of experience working with adults who have mental illness, particularly those individuals diagnosed with a serious and persistent mental illness preferred but not required
  • Communication, organization, time management and clinical skills
  • Ability to work independently in a community-based setting
  • Valid Colorado Driver’s license with a clean driving record
Responsibilities
  • Provide prevention/early intervention, care navigation, family psychoeducation, and other clinical interventions to consumers and families as appropriate to the position
  • Support and closely coordinate mental health care with the consumer’s clinical care team and the primary care provider and family members as needed
  • Provide brief interventions using evidence-based techniques such as Motivational Interviewing, de-escalation, or other treatments appropriate to support consumer engagement
  • Document initial intervention, track patient follow-up, and clinical outcomes using an Electronic Health Record system (EHR). Document in-person and telephone encounters in the EHR, and use the system to identify and re-engage patients
  • Attend collaborative treatment team meetings and reviews of consumers
  • Provide therapeutic crisis intervention and emergency services as needed
  • Assist eligible consumers in gaining access to needed medical, social, educational, and other services
  • Provide an assessment of needs, service plan development, referrals/linkage, and monitoring/followups for consumers
  • Facilitates and contributes to the accomplishment of tasks by consumers, empowers consumers to reach optimum level of achievement, and sets a positive example and provides role modeling, as well as technical expertise, to accomplish goals
  • Attend and participate in group sessions, in-service education and training opportunities, meetings, conferences,and special events in order to utilize opportunities for professional growth
  • Provide transportation for consumers in agency vehicles when clinically indicated
  • Work closely with outside organizations on resources and care coordination
  • Uses identified agency key performance indicators to track success, engagement, and other factors leading to successful completion of treatment
