Requirements
  • Availability to work Monday through Friday, 9am–6pm Eastern Time without restrictions.
  • Must be located in Ontario, Canada.
  • Must be authorized to work in Canada without employer sponsorship.
  • At least 1 year of member-facing healthcare experience, such as medical clinic front office, member registration, care coordination, enrollment, or telehealth outreach.
  • Ability to build trust and communicate effectively with older adults, including skeptical or hard-to-reach individuals.
  • Understanding of care coordination principles and common points where members become stuck.
  • Ability to discuss chronic conditions, medications, and treatment plans with members while reinforcing clinical guidance without providing medical advice.
  • Proficiency with electronic health record systems, care management platforms, customer relationship management systems, and digital communication tools, including navigating multiple systems during a live call.
  • Ability to follow compliance procedures and process gates without cutting corners under pressure.
  • Willingness and ability to work across enrollment, welcome visits, and care navigation according to daily business needs.
  • Ability to track personal performance and follow through on assigned enrollment, welcome visit, and care navigation work.
  • Comfort with ambiguity and rapidly evolving processes.
Responsibilities
  • Conduct outbound outreach to prospective program participants through a structured referral queue.
  • Introduce the program in plain, accessible language and communicate its benefits without unnecessary clinical jargon.
  • Build rapport with older adults and address concerns and objections.
  • Obtain required verbal consents individually and in sequence before scheduling proceeds.
  • Complete and document intake questionnaires for prospective members who wish to enroll.
  • Coordinate and schedule initiating provider appointments after successful enrollment.
  • Conduct welcome visits with newly enrolled members and set expectations for ongoing support.
  • Explain the ongoing care coordination process.
  • Provide condition-specific education tailored to members’ literacy levels, languages, and learning preferences.
  • Coach members on symptom monitoring, when to contact a doctor, and medication routines.
  • Use motivational interviewing techniques and provide culturally sensitive, trauma-informed care.
  • Complete or confirm initial social determinants of health screenings and flag immediate barriers for pod follow-up.
  • Document member information thoroughly to support seamless transitions into ongoing coordination.
  • Liaise between members, primary care providers, specialists, pharmacies, home health agencies, and community resources.
  • Coordinate with primary care offices on referrals, prior authorizations, and medication changes, following through until completion.
  • Support medication adherence by identifying barriers, educating members on proper use, and escalating discrepancies or concerns.
  • Help members access affordable medications through insurance benefit exploration, prescription discount programs, manufacturer coupons, and member assistance programs.
  • Help members access durable medical equipment, transportation services, meal programs, and other community resources.
  • Conduct structured social determinants of health screenings using validated tools to identify food insecurity, housing instability, transportation challenges, and financial strain.
  • Own the process from identifying a need through confirming its resolution and receipt of the service.
  • Build and maintain a regional resource directory and update changing program and eligibility information.
  • Escalate care navigation concerns to the team lead promptly and clearly.
  • Document every member interaction accurately in real time.
  • Follow consent and compliance procedures without exception.
  • Work across the care navigation system, customer relationship management system, Google Workspace, and Google Chat.
  • Respond to quality audits, coaching, and feedback, and report workflow or platform issues.
  • Support care gap closure, resource connections, escalation response, appointment adherence, and transportation arrangements.
  • Maintain complete, same-day documentation and consistent daily and weekly workload output across enrollment, coordination, and welcome visits.
Desired Qualifications
  • Experience working with Medicare-enrolled, dual-eligible, or older adult populations.
  • Experience conducting post-hospital or post-emergency-department transitional care calls.
  • Experience using social determinants of health screening tools or navigating community resources.
  • Bilingual ability, with Spanish preferred and other languages depending on the target population.
