Requirements
  • Formal MSN-Nurse Practitioner or Physician Assistant training and certification in the state of Ohio to practice medicine.
  • Graduation from an accredited Master of Science in Nursing/Advanced Practice Registered Nurse program or ARC-PA accredited program
  • Board certification from ANCC/NCCPA or other valid Nurse Practitioner/Physician Assistant accreditation body with a focus in family medicine (all ages)
  • Licensed in the state of Ohio
  • Current Drug Enforcement Administration registration
  • Knowledge of principles and practices of preventative medicine
  • Knowledge of state and federal laws pertaining to medicine and public health
  • Knowledge of the structure and function of community health centers
  • Skill in communicating effectively with patients and families to include culturally sensitive care
  • Skill in establishing and maintaining effective working relationships with other employees, patients and the general public
  • Commitment to AxessPointe mission, vision and values
  • Bilingual a plus
Responsibilities
  • Examines, diagnoses, and treats illnesses and injuries of patients throughout the lifespan.
  • Provides well-child, well-woman and well-man health maintenance exams to new and established patients.
  • Orders appropriate screening and diagnostic lab and radiographic exams.
  • Interprets results of physical exams and screening/diagnostic tests to implement appropriate care plans for the patient.
  • Prescribes medicines, therapeutic agents and treatment modalities as required by patient needs and in accordance with licensed prescriptive authority.
  • Supports health promotion, health education, health empowerment and disease prevention interventions for all patients.
  • Provides age-appropriate care to all patients.
  • Promotes patient and family centered care through a collaborative team approach.
  • Participates in established call group as necessary.
  • Refers patients to specialty practices as appropriate and needed. Provides medical care to patient as part of mutual patient care with specialist.
  • Confers with collaborating physician about patient care as necessary.
  • Documents and maintains patient records of services provided according to program standards.
  • May instruct medical students or nursing students.
  • Operates within a scope of practice as agreed to by the FNP/PA, the collaborating physician, the medical director and in compliance with Ohio state law.
