Requirements
  • High school education and 2–5 years of experience, or an Associate's degree and 1–3 years of experience, or a Bachelor's degree and 0–1 year of experience.
  • A New York State motor vehicle license, a safe driving record, and availability of a personal vehicle for work are required.
  • Qualified Mental Health Staff status, as defined by Office of Mental Health Residential regulations, is required for work in Office of Mental Health-licensed programs.
  • Provisional Qualified Health Professional status or CASAC-T is required in Office of Addiction Services and Supports-licensed programs. Counselors hired with Provisional Qualified Health Professional status must obtain their CASAC or other Qualified Health Professional status within three years.
  • Narcan training is required for Office of Addiction Services and Supports-licensed or funded programs.
  • Effective written and oral communication skills are required.
  • Ability to work cooperatively and effectively with culturally diverse clients, staff, and community service providers is required.
  • Basic computer literacy is required, including conducting basic online searches, checking email, creating and sending email with an attachment, using basic email features, and opening, reading, and making simple edits to spreadsheets and word-processing documents.
  • Basic Microsoft Word skills are required, including producing basic business letters and simple tables and charts; checking spelling; setting tabs; changing page orientation; sorting; and saving, opening, and organizing electronic files.
Responsibilities
  • Provide comprehensive care management and coordinate direct care and support services, basic-needs assistance, and continuity of care, using appropriate interventions based on guests' goals and documenting their progress.
  • Partner with guests to develop and pursue individualized service-plan goals based on guest-identified objectives and preferences, including education and support for self-management, self-help recovery, and other appropriate resources.
  • Maintain guest and facility safety through regular environmental monitoring and crisis response.
  • Complete comprehensive guest assessments, including mental health and substance-use-disorder assessments, risk assessments, health screenings, and crisis planning.
  • Collaborate with guests to develop psychiatric advance directives so their wishes are known and can be followed during a crisis.
  • Conduct check-in and check-out activities and facilitate groups and activities.
  • Coordinate client services within the community network; advocate for mental health, chemical dependency, medical, financial, legal, social, career, housing, and recreational services; coordinate transportation as needed; and conduct outreach to encourage guest participation and support.
  • Develop and maintain effective relationships with landlords, community groups, referral sources, and service agencies.
  • Participate in regular case reviews to track care-plan goals and make changes as needed.
  • Maintain knowledge of evidence-based practices and community-centered resources, and receive training or higher education to augment the skill set.
  • Design and conduct weekly group meetings tailored to current guest needs, applying a holistic approach to support clients.
  • Design and implement in-house guest-enrichment activities to foster peer connections, community integration, mutual support, and social engagement.
  • Coordinate activities and maintain appropriate schedules for the location.
  • Collaborate with community agencies to identify resources and connect guests to community resources and new or existing healthcare providers.
  • Assist guests with securing basic needs, including food, clothing, and housing.
  • Maintain organized facility resources and an active resource for others to use when assisting guests during a crisis.
  • Facilitate trainings, workshops, groups, and activities as needed, including enrichment groups on life skills, community resources, conflict resolution, and self-resiliency.
  • Provide outreach to clients and ensure continuous support after discharge.
  • Assist in daily residence operations, including light housekeeping, cooking, and purchasing food and supplies.
  • Supervise and coach clients in functional independent-living skills, including cleaning, meal preparation, and maintaining safe and sanitary conditions in the group home or apartment.
  • Supervise clients in managing their medications and comply with East House medication policies and procedures.
  • Meet medication documentation and supervision standards and avoid medication errors.
  • Maintain documentation in accordance with regulator, funder, and East House standards and policies; meet standards as evidenced by audits, chart reviews, and recertification reviews.
  • Follow applicable policies, federal and state laws, and East House compliance standards.
  • Collaborate with the Office of Ethics, Compliance & Quality to address non-compliance and document corrective actions.
  • Assess, monitor, and evaluate needs and progress in the program.
  • Travel to various program locations as needed.
