Requirements
  • A high school diploma or GED is required.
  • Two or more years of experience working for a healthcare practice or hospital are required.
  • The ability to lift up to 20 pounds is required; moving, lifting, or transferring patients may involve lifting up to 50 pounds and assisting with weights exceeding 100 pounds.
  • The ability to stand for extended periods is required.
  • The ability to drive to patient locations such as homes, hospitals, and skilled nursing facilities is required.
  • Fine motor skills and visual acuity are required.
  • Strong critical thinking skills are required.
  • A strong customer-centric focus and service skills are required.
  • Strong computer skills, including Microsoft Word, Excel, and PowerPoint, are required.

Responsibilities
  • Update and maintain digital client records and assist with transitioning hard-copy records to digital format.
  • Act as a community resource for patients.
  • Conduct patient scheduling, registration, and eligibility verification.
  • Manage patient medical records as appropriate.
  • Answer patient phone calls and emails, make outgoing calls, and prioritize patient concerns as appropriate.
  • Answer patients’ non-clinical questions and explain processes.
  • Schedule and coordinate WellBe provider visits, tests, specialist appointments, and social resources with patients.
  • Ensure all appropriate patient information is entered in the electronic medical record.
  • Provide customer service and treat patients and staff with respect.
  • Prioritize urgent patient needs, scheduling needs for Complete Care Assessment, and efforts to engage unengaged patients.
  • Collaborate with the interdisciplinary team and participate in regularly scheduled team meetings.
  • Support advanced practicing clinicians in ongoing panel management and quality-of-care efforts, including Healthcare Effectiveness Data and Information Set activities.
  • Complete next-day visit chart preparation.
  • Use reporting to help providers track and meet team goals.
  • Perform other tasks needed to accomplish the team’s objectives and goals.
Desired Qualifications
  • Medical assistant certification is preferred.
  • Electronic medical record documentation experience is preferred.

WellBe Senior Medical has appointed four new leadership team members as the home-based medical care provider scales operations across the US. The company now serves over 120,000 patients in eight states. The new hires include Shafina Shehnaz as chief financial officer, bringing experience from Optum Health where she served as CFO of the care delivery arm. Sabita Lahiri joins as chief transformation officer after leading clinical strategy at Landmark Health for seven years. Dr Katrina Booth, a board-certified geriatrician, becomes chief medical officer, whilst Cammile Bird takes on the role of senior vice president of engagement. CEO Dr Jeff Kang said the appointments will help deliver personalised at-home care as WellBe enters "a pivotal chapter of growth".
