Requirements
  • The candidate must be a graduate of an accredited School of Nursing.
  • The candidate must hold current appropriate state licensure and meet the practice requirements in the state of employment.
  • The candidate must hold or successfully complete CPR Basic Life Support Certification.
  • The candidate must have at least 12 months of experience as a Registered Nurse to provide assigned oversight after completing the required training.
  • The candidate must have three months of experience as a Registered Nurse in home peritoneal dialysis and/or hemodialysis to provide assigned oversight after completing the required training.
  • The role is entry level for Registered Nurses with less than two years of Nephrology Nursing experience.
  • The candidate must be able to perform direct patient care involving heavy lifting, moving equipment and patients, and assisting with ambulation.
  • The candidate must be able to stand for prolonged periods, bend over, and occasionally move machines and equipment weighing up to 200 pounds with assistance.
  • The candidate must be able to lift chemical and water solutions weighing up to 30 pounds as high as five feet.
  • The candidate must participate in an on-call rotation, including nights, weekends, holidays, or other program-defined coverage.
  • The candidate may be required to travel to training sites, other facilities, or patient homes.
Responsibilities
  • Provide and coordinate clinically competent care, including assessment, planning, intervention, and evaluation, for an assigned group of patients.
  • Assess and manage patients’ responses to home dialysis training and treatment therapy according to prescribed protocols, and communicate patient-related issues to the physician as needed.
  • Perform ongoing collection and analysis of dialysis data for assigned patients, document it in the medical record, modify treatment plans when indicated, and notify the Supervisor or physician as needed.
  • Assess, collaborate on, and document patients’ and families’ basic learning needs to provide initial and ongoing education.
  • Direct and provide, in collaboration with patients, home care partners, and care staff, all aspects of safe and effective dialysis therapy delivery.
  • Administer prescribed medications or medications authorized under approved algorithms, and document medical justification and effectiveness.
  • Initiate or assist with emergency response measures.
  • Serve as a resource for the health care team and participate in staff training and orientation of new staff as assigned.
  • Ensure correct laboratory collection, processing, and shipping procedures, and reschedule missed or insufficient collections.
  • Identify expected outcomes and document and update nursing assessments and plans of care in collaboration with the Interdisciplinary Team.
  • Ensure patient awareness of transplant and treatment modality options.
  • Assist in identifying, evaluating, selecting, and educating Home Dialysis candidates and Home Partners.
  • Assess the home environment and partner or family readiness and ability to perform dialysis treatments.
  • Train Home Dialysis patients and/or Home Partners in the safe and effective operation and maintenance of Home Dialysis equipment and treatment supplies through a formal training program.
  • Complete Clinical Advancement Program requirements to advance.
  • Rotate coverage with other licensed home therapies staff to ensure reliable and adequate coverage.
  • Perform other duties assigned by the Supervisor.

  • At least one year of experience as a Registered Nurse is preferred.
  • Home dialysis therapy experience is preferred.
