Requirements
  • A high school diploma or GED is required.
Responsibilities
  • Provide habilitation services to each client as prescribed in that client’s service plans, including CFA, Individual Service Plan, and Habilitation Plan.
  • Assist each client, as needed, with skill development in activities of daily living, domestic living, recreation, and socialization.
  • With appropriate training and supervision, administer medication to clients as assigned and in compliance with management, medical, and nursing policies and procedures.
  • Ensure assigned clients’ nutritional needs are met according to the prescribed diet, and assist with feeding as necessary.
  • Immediately report observed or known deficiencies, inability to provide habilitation services, abuse, facility problems, and physical plant or safety issues to management.
Desired Qualifications
  • One year’s experience providing direct services to developmentally disabled persons.
  • An associate degree.
  • Preferred certifications: AMAP, SCIP-R, CPR, and First Aid.

It coordinates services such as case management, housing and stabilization, behavioral health treatment, recovery supports, rehabilitation, advocacy, and community-based programs tailored to each person’s needs. By linking housing, health, justice, and community resources, S:US delivers integrated, place-based services that address multiple barriers at once rather than focusing on a single issue. The goal is to help people lead purposeful lives, improve neighborhoods, and break the cycle of poverty for current and future generations of New Yorkers.
