About the job
- A valid California Certified Home Health Aide certification is required.
- A current Basic Life Support or cardiopulmonary resuscitation certification is required.
- A valid California driver's license is required.
- Candidates must complete a computer literacy and competency assessment before an in-person interview, covering electronic charting, company values, and use of the web-based communication application.
- Candidates must be able to build rapport with patients and provide high-quality care.
- Candidates should be self-directed, empathetic, able to think critically, and organized, with strong interpersonal skills.
- Provide patient care according to the Plan of Care under the direct supervision of an RN Case Manager.
- Provide personal care, including baths, back rubs, oral hygiene, shampoos, and changing bed linen.
- Assist patients with dressing and undressing.
- Provide skin care to prevent breakdown.
- Assist patients with toileting activities.
- Keep the patient's living area clean and orderly, as appropriate.
- Assist with feeding the patient, if necessary.
- Perform range-of-motion and other simple procedures as an extensional therapy service when ordered and competency has been completed or demonstrated.
- Assist patients with self-administration of medication.
- Meet patients' safety needs and use equipment safely and properly, including foot stools and side rails.
- Report patients' conditions and significant changes to the assigned nurse.
- Visit patients in their homes as required.
- Experience in home health care is highly preferred.
- Experience with high-acuity patients is a plus.
- Experience working with an interdisciplinary team is highly desired.
