Requirements
  • Be a Registered Nurse.
  • Have at least one year of experience as a registered nurse in acute care hospital, oncology, long-term care, hospice, or home health; school internships and clinical hours may be considered in individual cases.
  • Hold a current license to practice nursing in the state of employment.
  • Hold current cardiopulmonary resuscitation certification.
  • Have current automobile insurance, a valid driver's license, and transportation for work.
  • Have knowledge of terminally ill patients and their families, hospice services, the roles of hospice disciplines, community resources, general nursing practice, managed care principles, and applicable regulatory guidelines, including Medicare, Medicaid, and ACHC.
  • Demonstrate patient assessment, clinical documentation, oral and written communication, critical thinking, adaptability, and the ability to support patients and families during crisis.
  • Effectively operate nursing assessment tools, computers, facsimile equipment, copiers, and cell phones or pagers.
Responsibilities
  • Serve as the primary point of contact for hospice patients and families throughout their hospice journey.
  • Deliver and document skilled nursing care according to individualized plans of care in patients' homes, assisted living facilities, or other care settings.
  • Perform comprehensive initial and ongoing patient assessments and reassessments, including in-person and telephonic registered nurse assessments and screenings.
  • Deliver skilled nursing interventions and symptom management aligned with goals of care.
  • Develop and update individualized plans of care, including visit frequency and risk identification.
  • Collaborate with physicians, licensed practical nurses, certified nursing assistants, social workers, chaplains, nurse practitioners, and other interdisciplinary team members to coordinate care and refine care plans.
  • Provide emotional, physical, and spiritual support, education, comfort, and bereavement guidance to patients and loved ones.
  • Teach patients, families, and caregivers about disease processes, self-care, end-of-life care, medication administration, nutrition, dietary needs, hospice philosophy, symptom and pain management, and end-of-life processes.
  • Communicate changes in patient condition and signs or symptoms of decline to the appropriate provider.
  • Support home health, palliative care, and hospice referrals and transitions between levels of care.
  • Maintain timely, accurate, and complete clinical records, including screening and assessment findings, physician orders, progress notes, and care plans.
  • Maintain effective working relationships with the interdisciplinary group and involve the team when the patient's care plan requires team participation.
  • Participate in on-call and patient support activities and conduct on-call services in a clinically competent and responsive manner.
  • Follow infection-control, regulatory, compliance, and organizational policy standards.
  • Support quality improvement and scheduling initiatives.
