  • A master's degree from an accredited school of Social Work, Mental Health, Psychology, or Marriage and Family Therapy is required.
  • Two years of direct clinical experience with an emphasis on the population served in the assigned area are required; successful completion of a master's-level internship within the population served may substitute for the experience.
  • Handle with Care certification is required for the Behavioral Health Unit.
  • Ability to prioritize work and work independently.
  • Knowledge and skills necessary to provide care appropriate to the age of patients served, including principles of growth and development across the life span.
  • Awareness of medical and legal issues, patient rights, and compliance with regulatory and accrediting standards.
Responsibilities
  • Collaborate with the assigned clinical team to identify patients who would benefit from care coordination, assess risk factors and care coordination needs, and support clinical utilization management and preventive care services.
  • Ensure continuity and consistency of care across inpatient, emergency, ambulatory, and outpatient settings, including comprehensive discharge planning and outpatient follow-up care.
  • Develop working relationships with Patient and Family Counselors, Social Workers, and utilization review nurses to engage patients and families, advocate, solve problems, and support timely discharge planning.
  • Monitor progress toward discharge plans daily and revise plans when patient condition or family needs change, prioritizing patients at highest risk for complications, admission, or readmission.
  • Educate patients and families with chronic illness about evidence-based standards of care and self-management strategies.
  • Identify patient and family support needs, develop action plans, and provide guidance to initiate and overcome self-management challenges.
  • Educate patients and families about the healthcare system and facilitate relationships among care settings.
  • Ensure patients have access to prescriptions, durable medical equipment, and other identified services.
  • Contribute to team problem solving through communication, collaboration, data collection, consensus building, evaluation of treatment options, and tracking and revising care-plan goals.
  • Advocate for patients' healthcare needs, including safety, physical, legal, and financial well-being.
  • Refer patients for education about healthcare delivery and reimbursement systems, prescription drug programs, health and wellness programs, community agencies, public and private organizations, housing options, and other services.
  • Use available information technology resources, including Phytel and Crimson, to support registry reporting and maintenance for specified patient populations through evidence-based guidelines, clinical decision-support tools, referral and test tracking, and preventive-medicine reminders.
  • Participate in clinical outcome measurement and identify strategies that promote population health.
  • Implement policies, procedures, and standards supporting assigned duties and patient safety.
  • Provide clinical treatment interventions under the supervision of a licensed mental health therapist, including facilitating patients' psychosocial adjustment across the continuum of care and transition to the next level of care.
  • Participate in psychosocial support groups.
  • Provide mental health education, information, consultation, and support for patients and families.
  • Maintain regular and punctual attendance and comply with organizational policies and applicable legal and regulatory standards.
  • Perform other assigned or required duties.

Florida cancer centre opens 31,000 sq ft St Petersburg facility with on-site imaging

Florida Cancer Specialists & Research Institute has opened a new 31,000-square-foot facility at Orlando Health Institute Square in St. Petersburg, expanding cancer care access in Pinellas County. The location at 725 6th Ave South features 18 private exam rooms, 47 infusion chairs and on-site PET/CT imaging. The facility offers advanced treatments including bispecific therapies and clinical trial participation, with plans to expand outpatient cell therapy programmes.
