- Perform in-home patient assessments.

- Provide skilled nursing care, including wound care, medication administration, catheter care, and chronic disease management.
- Educate patients and caregivers on home care and safety.
- Monitor patient progress and update care plans.
- Complete timely and accurate documentation.
