Requirements
  • A Master’s Degree.
  • National Board Certification as an Acute Care Nurse Practitioner or Physician Assistant.
  • Basic Life Support certification.
Responsibilities
  • Work within the ambulatory setting in collaboration with a multidisciplinary health care team and provide comprehensive care in the Gastroenterology ambulatory practice.
  • Perform complete histories and physicals and document findings in the medical record.
  • Order and collect data using appropriate assessment techniques, supporting diagnostic information, and diagnostic procedures when indicated.
  • Serve as a first responder for nursing and house staff during patient status changes and situations requiring orders, procedural interventions, or resuscitative interventions.
  • Assess risks associated with care of acute and complex chronically ill patients, including medication side effects, immobility, impaired nutrition, fluid and electrolyte imbalance, immunocompetence, invasive interventions, and diagnostic procedures.
  • Collaborate with the interdisciplinary health care team to diagnose acute and complex chronic conditions.
  • Manage diagnostic tests through ordering, interpretation, performance, and supervision.
  • Formulate differential diagnoses by priority.
  • Diagnose complications and order appropriate interventions.
  • Identify expected outcomes from diagnoses and formulate and document individualized plans of care for complex acute and chronic health care needs.
  • Use evidence-based practice guidelines and protocols in dynamic plans of care across the continuum.
  • Implement and modify plans of care.
  • Prescribe diagnostic strategies and therapeutic interventions.
  • Perform advanced procedures consistent with privileges and competency validation when applicable.
  • Document or dictate patient progress, including histories and physicals, daily progress notes, plans of care, problem lists, procedure notes, acute event notes, discharge summaries, clinic notes, transfer summaries, and death summaries, in the medical record as required by the patient unit or service.
  • Maintain timely documentation that meets acute-care compliance standards and captures patient acuity.
  • Maintain ongoing communication and collaboration with the interdisciplinary health care team.
  • Facilitate communication with patients, families, and staff to promote continuity of care across inpatient and outpatient settings.
  • Provide precepting, mentoring, education, and instruction to graduate students.
  • Seek opportunities for active engagement in research and analysis of evidence-based practice.
  • Participate in Advanced Practice Grand Rounds and specialty-specific meetings.
  • Maintain continuing education units and membership in a professional organization.
