Requirements
  • A bachelor's degree in a behavioral health field from an accredited institution and one year of relevant post-degree experience; alternatively, an associate's degree in a direct care field, such as psychosocial rehabilitation or psychiatric nursing, from an accredited institution and two years of relevant post-degree experience may substitute for the bachelor's degree; alternatively, four years of relevant work experience with individuals who are or have been primary consumers of mental health services is required.
  • A genuine desire to provide in vivo Integrated Case Management Services and comfort moving about in consumers' communities.
  • Ability to work a flexible schedule, including evening, weekend, and holiday hours.
  • A valid driver's license.
Responsibilities
  • Establish and maintain ongoing therapeutic relationships and case manage up to 20 consumers with mixed risk levels.
  • Provide intensive community-based services to maximize consumers' access to services, ability to function adequately, and integration into the community; provide holiday, weekend, and on-call coverage as assigned.
  • Develop and implement service plans and complete required documentation.
  • Facilitate access to appropriate services, including transportation to services and activities as necessary and applications for and receipt of applicable public entitlements.
  • Coordinate and integrate services from multiple providers until the consumer is discharged from Integrated Case Management Services, including coordinating meetings with the consumer's community service providers.
  • Monitor service delivery to meet consumers' changing needs and advocate as necessary.
  • Identify consumer resource gaps and service delivery problems, and provide direct service support to consumers' natural support systems, including family, friends, employers, and self-help groups.
  • Maintain records required by funding sources and accreditation agencies, including case notes, group notes, progress notes, service planning, termination summaries, correspondence with insurance providers, and outcome data.
  • Meet expected productivity levels for direct service provision to consumers.
  • Perform other duties as assigned by the Program Director.
