Licensed Practical Nurse

Job Description

Home Health Care Solutions is seeking a Full-Time LPN (Licensed Practical Nurse) to provide skilled nursing care to patients within the LaPorte area. Estimated pay is $62,000 to $65,000+ per year. Pay is based on completing 30 units per week, which is estimated to be approximately 40 hours of work per week. There is an opportunity to earn more by working more than 30 units per week.

Key Responsibilities

• Provide skilled nursing care and case management in a home health setting.
• Collaborate with an interdisciplinary team to develop and implement individualized care plans.
• Perform patient and family education to promote independence and optimal clinical outcomes.
• Maintain accurate and timely clinical documentation.

Requirements

• License: Current LPN license in the State of Indiana.
• Skills: Strong clinical assessment, communication, and organizational skills.
• Experience: Home health experience is preferred but not required.
• Reliability: Valid driver’s license and dependable transportation for travel within the LaPorte territory.

Qualifications

• Graduate of an accredited school of nursing
• Valid nursing license for the State of Indiana required   
• One year clinical experience in nursing
• Home Care or Critical Care nursing preferred
