Requirements
- A Bachelor's degree in Pharmacy or Doctor of Pharmacy (PharmD) degree is required.
- Must be at least 18 years of age and have no current drug or alcohol addiction or convictions of any drug-related misdemeanor or felony.
- Current active Pharmacy Licensure (PH) for the work state is required.
- Mastery of the Cerner pharmacy and hospital information system application is required.
- Must be proficient with technology and workflows associated with the medication management process, including automated dispensing cabinets, anesthesia carts, USP <797> IV requirements, and clinical decision support software.
- Maintain competency in pharmaceutical knowledge and skills.
- Effective verbal and written communication skills.
- Ability to prioritize time and tasks according to patient needs and departmental duties.

Responsibilities
- Provide medication therapy management services in collaboration with other providers to ensure drug therapy is safe, appropriate, cost-effective, and efficient.
- Maintain competency in unique aspects of drug therapy for patients of all age groups, including neonates, pediatrics, adults, and geriatrics, and patient dispositions, including inpatient, ambulatory, and outpatient.
- Provide pharmacy services that may include interdisciplinary patient care rounds, medication reconciliation, medication therapy monitoring, medication orders management, medication dispensing, patient education, drug information, drug dosing, and scholarly activities.
- Perform clinical and distributive tasks for safe medication use, and provide training and education to students and residents as necessary.
- Review medication orders for appropriate dose, duration, frequency, dosage form, indication, allergy, interactions, and contraindications.
- Ensure medication regimens are consistent with organizational medication-use guidelines and policies, or evidence-based guidelines when organizational guidelines are absent, including antimicrobial use, intravenous-to-oral transition, renal dosing, anticoagulation, and pharmacokinetics/serum drug concentration protocols.
- Adjust medication regimens based on approved protocols and/or consultation with the interdisciplinary team, and monitor medication therapy for potential or actual drug interactions and outcomes.
- Work as an active member of the multidisciplinary team and collaborate with healthcare providers in decentralized patient-care areas to provide patient-centered care.
- Participate in interdisciplinary patient-care rounds and oversee pharmacy resident and student participation in rounds when applicable.
- Perform and/or review medication histories by comparing prescribers’ orders to patients’ active medication lists, and work with prescribers to make changes as necessary.
- Document medication-reconciliation activities in the medical record across transitions of care, including admission, transfer, and discharge.
- Complete discharge medication reviews and facilitate discharge, including discharge prescriptions, and provide discharge education per department procedures or upon request.
- Ensure compliance with the health system inpatient drug formulary.
- Complete medication-profile reviews when staffing clinical areas, considering age, sex, health, weight, renal and hepatic function, other physiological parameters, pregnancy and lactation status, dose, route, rate, timing, frequency, indication, therapeutic duplication, laboratory values, microbiology results, antimicrobial therapy and planned stop dates, potential drug interactions, and contraindications.
- Coordinate and participate in medication preparation and distribution, including communicating pending issues and transactions with team members and other healthcare professionals.
- Coordinate drug delivery and documentation as appropriate.
- Complete and pass competencies for sterile, non-sterile, and hazardous drug compounding management, as assigned based on the work area.
- Follow safety precautions, including hazardous-material handling and precautions against blood-borne pathogens, as related to the area of practice.
- Monitor the medication-use system to ensure the right patient, drug, dose, route, and time, and the right outcomes.
- Serve as a liaison for inpatient patient information concerning outlying patient needs, including retail pharmacies, home health, and physician clinics.
- Identify and report adverse drug events, including adverse drug reactions and medication errors, in the organizational reporting system.
- Ensure compliance with health-system, hospital, and departmental policies and procedures.
- Monitor compliance with state and federal laws and regulations related to pharmacy practice and standards and regulations of other regulatory agencies.
- Participate in departmental and hospital committees as assigned.
- Provide direct supervision of certified pharmacy technicians, pharmacist interns, and pharmacy students, and assist with their education, training, and development.
- Provide drug information and education to healthcare providers and patients upon request and according to department procedures.
- Provide in-services and lectures to healthcare team members as assigned.
- Precept and evaluate pharmacy residents and PharmD students on residency and clerkship rotations.
- Provide drug information and disease-prevention programs to consumer and patient groups as needed.

- Complete required departmental education and competency modules.
Desired Qualifications
- Two years of healthcare pharmacy experience is preferred.
- American Heart Association Basic Life Support (BLS) certification is preferred.
- American Heart Association Advanced Cardiovascular Life Support (ACLS) certification is preferred.
- Completion of an ASHP-accredited residency training program and/or certification through a certifying program, such as Board of Pharmacy Specialties, is preferred.
