Requirements
  • New York State Licensed Practical Nurse.
  • A minimum of two years of relevant experience, including knowledge of the population served.
  • Current CPR certification.
  • Work experience relevant to degree and certification requirements.
  • Ability to safeguard confidential information.
  • Commitment to maintaining and pursuing continuing professional education.
Responsibilities
  • Provide nursing services to residents under the direction of a Registered Nurse.
  • Monitor the facility to ascertain residents’ activities and the general status of the facility.
  • Initiate appropriate action to ensure the safety and well-being of residents and the facility, and document significant activity in the clinical log.
  • Check residents identified as needing monitoring because of physical or emotional difficulties, initiate action based on assessment, and document findings in the clinical log.
  • Offer crisis-intervention counseling to residents as needed, document sessions in the staff log, and report to the supervisor.
  • Complete resident incident, accident, medical emergency, and other required forms.
  • Answer phones and provide support to people needing assistance.
  • Meet regularly with the Registered Nurse and Health Coordinator for supervision according to policy.
  • Assess and monitor residents for signs and symptoms of withdrawal using appropriate withdrawal scales.
  • Perform other duties as assigned.
