Clinical Nurse Manager: 

• Manage the delivery of our high-quality ambulatory clinic care and oversee a high-performing, well-equipped clinic team ensuring workflow efficiency, clarity, and protocols

RN Case Manager:

• Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each participant’s unique needs

Clinic RN:

Enrollment RN:

• Conduct thorough assessments to assess eligibility and guide seniors and their families through the enrollment process for our PACE program

Home Health RN:

RN Job Requirements:

• Graduation from an accredited nursing program and a minimum of one (1) year of experience as an RN
• Current and unencumbered RN license in California
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal abilities
• Prior experience in senior healthcare or PACE preferred, but not required
