Requirements
  • A master's degree in Social Work.
  • A state-required social work license.
  • Two to five years of related experience.
  • Knowledge of social work theory, accepted social work practice methods, psychosocial interventions, quality-of-life measurement instruments, advance directives, DNR discussions, dialysis treatment modalities, End Stage Renal Disease support services, insurance programs, financial assistance programs, and community resources.
  • Ability to work within state regulatory caseload guidelines.
  • Ability to travel when working across multiple facilities.
Responsibilities
  • Assess patients' psychosocial status, strengths, and areas of need as part of comprehensive patient assessments.
  • Participate in care planning with patients and the healthcare team to identify interventions supporting rehabilitation, treatment goals, and quality of life.
  • Use patient education programs, social work theory and methods, focused interventions, and quality-of-life measurement instruments in assessment and care planning.
  • Monitor patients and provide interventions to support adjustment to dialysis and optimal psychosocial status and quality of life.
  • Provide supportive, educational, and goal-directed counseling within the scope of clinical training and state licensure.
  • Provide education and counseling to patients seeking transplantation.
  • Refer patients to community resources, vocational rehabilitation, educational rehabilitation, home health services, and other support programs.
  • Assess awareness of advance directives and assist patients and families with forms, information, and discussions of advance directive wishes.
  • Participate with physicians and nurses in discussions of patient do-not-resuscitate status to support informed decisions.
  • Document social work interactions and interventions provided to patients and families.
  • Provide psychosocial support and social work-focused interventions addressing non-adherence, quality outcomes, and quality-of-life concerns.
  • Participate in monthly quality review meetings and report on adherence, treatment, quality-of-life, and service-recovery indicators.
  • Assess patient knowledge of kidney disease and provide education tailored to learning style, communication barriers, and individual needs.
  • Provide information about available treatment modalities with the interdisciplinary team.
  • Facilitate the transplant referral process and collaborate on transplant waitlist management.
  • Provide ongoing education about psychosocial issues related to End Stage Renal Disease and available support services.
  • Review patient rights, responsibilities, grievance information, nondiscrimination policies, DNR information, and facility policies with patients or representatives.
  • Support the patient admission and referral process, including scheduling needs that promote adherence and adjustment.
  • Interview patients requesting discharge to identify root causes or concerns and share them with operational leadership.
  • Liaise with insurance coordinators regarding insurance issues, payment information, coverage changes, transplantation, discharge, and extended travel.
  • Refer patients to the patient billing solutions department for treatment-billing questions and concerns.
  • Assist with interviews and hiring decisions for new personnel when requested by the Social Work Manager or Senior Manager.
  • Maintain current patient resource lists with administrative support staff.
  • Provide psychosocial training to staff as needed.
  • Participate in weekly team huddles and discuss urgent patient issues with staff.
  • Perform other related duties as assigned.
