- Collaborate with an interdisciplinary team of nurses, social workers, chaplains, certified nursing assistants, and volunteers to develop care plans supporting patients' and families' physical, emotional, and spiritual needs.
- Manage daily scheduling of patient visits, documentation, meetings, and urgent needs.
- Travel throughout the service area to provide patient care.
