About the job
- Graduate of an accredited diploma, associate, or baccalaureate school of nursing.
- Hold a current state license as a Registered Nurse.
- Have a minimum of 1 year of experience as a licensed Registered Nurse in a clinical care setting.
- Have reliable transportation, a valid driver's license, and valid automobile insurance coverage.
- Be willing to travel daily for home visits within the service area.
- Demonstrate clinical assessment and critical thinking skills.
- Be proficient in OASIS documentation and care planning.
- Have effective communication and teaching skills.
- Be able to work independently with minimal supervision and in diverse home and facility environments.
- Demonstrate clinical judgment and decision-making capabilities.
- Understand Medicare, Medicaid, and third-party payor requirements.
- Deliver comprehensive skilled nursing care to patients in their homes, supporting recovery, chronic disease management, and patients' health goals.
- Manage a caseload of patients from admission through discharge.
- Conduct assessments and develop individualized care plans tailored to each patient's needs.
- Work closely with physicians and an interdisciplinary care team to coordinate services and make informed clinical decisions.
- Provide care to patients in their homes, assisted living facilities, or nursing homes through daily travel within the service area.
- Bachelor of Science in Nursing (BSN).
- Experience in a home health environment.
- OASIS certification or willingness to obtain it.
