Requirements
  • A Certified Nursing Assistant (CNA) license is required.
  • At least 12 months of caregiving experience in home care, assisted living or memory care, skilled nursing or nursing home, hospital, or hospice settings is required.
  • Current CPR certification is preferred.
  • The candidate must have customer service skills.
  • The candidate must have written, verbal, and active-listening communication skills.

Responsibilities
  • Perform personal care activities related to daily living, including bathing, dressing, grooming, toileting, and skin care.
  • Review patient care plans.
  • Assist with lifting and transferring patients.
  • Assist with range of motion, positioning, exercise, and ambulation.
  • Assist with dining and nutrition.
  • Maintain electronic medical records.
  • Work under the direction of a registered nurse Case Manager.

Moments Hospice provides Medicare-certified in-home hospice and palliative care across Minnesota, Wisconsin, Iowa, and select metros, delivering services wherever the patient calls home. It develops a joint, individualized care plan within seven days in collaboration with the patient, family, and treating facilities, and offers routine home care, continuous crisis care, general inpatient care, respite care, plus bereavement and spiritual support. The Moments Way centers on coordinating care with patients, families, and referral partners and uses a holistic approach to address physical, mental, and spiritual needs. Its goal is to improve quality of life for terminally ill patients, support families through grief, and raise the standard of hospice care, aided by the Moments Hospice Foundation that fulfills final wishes.
