Requirements
  • Registered Physiotherapist in good standing with the College of Physiotherapists of Ontario (required)
  • Valid G2 or G driver’s license with access to a reliable vehicle for community travel
  • Community-based physiotherapy experience is an asset, however new graduates are welcome
  • Ability to work independently while collaborating within an inter-professional team
  • Strong organizational skills, including self-scheduling, prioritization, and meeting reporting deadlines
  • Excellent verbal, written, and report-writing communication skills
  • Proficiency with computer applications, including MS Office Suite and proprietary clinical software
Responsibilities
  • Provide ministry-funded, community-based physiotherapy services within clients’ homes and/or through virtual service delivery
  • Complete comprehensive physiotherapy assessments using interviews, standardized tools, and informal assessment techniques in areas including: Geriatrics; Neurology; Orthopedics; Palliative care
  • Develop and implement goal-oriented, evidence-based treatment plans in collaboration with clients and caregivers
  • Assign, supervise, and monitor physiotherapy assistants, when appropriate
  • Provide education to clients and caregivers and connect them to appropriate community resources
  • Maintain accurate, timely, and professional documentation, including: Assessment findings; Care plans; Progress and discharge summaries; Documentation aligned with referral source requirements and College of Physiotherapists of Ontario (CPO) standards
