Requirements
  • A bachelor's degree in social work or public health, or at least two years of experience providing outreach, navigation, and low-threshold to intensive case management services to high-risk populations, including expert knowledge of stabilization, shelter, housing, and substance use disorder treatment centers.
  • Experience and training in de-escalation, motivational interviewing, overdose reversal, data collection, and case notes.
  • Experience providing street-based outreach services in urban settings and homeless encampments.
  • Ability to read, write, collect data, and document case notes to expected professional standards.
  • Ability to establish and maintain effective collaborative relationships across GLIDE programs and with city agencies across San Francisco, the wider Bay Area, California, and the United States.
  • Ability to engage clients in services and retention through motivational interviewing, risk reduction counseling, and strengths-based counseling techniques.
  • Understanding of the social determinants of health and their impact on people experiencing homelessness, actively engaged in drug use, and proximate stressors.
  • Ability to provide radically inclusive, unconditionally loving, culturally competent, and trauma-informed support to diverse clients to help them achieve stability and self-sufficiency.
  • Strong communication and trust-building skills, including comfort and proficiency with public speaking in one-to-one client and community interactions, group education, and street outreach.
  • A valid California driver's license or the ability to acquire one within 60 days of hire.
  • Auto insurance and a clean DMV record, with the ability to drive GLIDE's Journey Home Service Van.
  • California Peer-to-Peer Support Specialist certification or the ability to acquire it within 60 days of hire.
  • Ability to complete the required California Peer-to-Peer Support Specialist certification within 60 days of hire.
  • Ability to work overnight shifts, remain flexible for schedule changes and emergency assignments, and work special events when required.
  • Ability to meet the physical requirements of the role, including walking, standing, navigating office spaces, and occasionally carrying or lifting up to 50 pounds.
Responsibilities
  • Conduct outreach, educate, and connect people experiencing or at risk of homelessness to shelter, housing, and coordinated-care wrap-around services.
  • Document and maintain complete case files for 20–25 active clients, including intakes, assessments, case management notes, appointment and attendance logs, engagements, referrals, connections, and other required data.
  • Submit daily, weekly, and monthly activity and data reports capturing contract deliverables in required database systems by their due dates.
  • Transport clients to and from appointments, arrange travel, and track, manage, and distribute client resources according to best practices for preventing theft, fraud, misuse, and abuse.
  • Follow professional boundaries, case-management ethics and values, HIPAA protocols, approved public-speaking guidelines, and other standard operating procedures.
  • Build and maintain positive rapport with clients, maintain an active caseload, and support client progression within the coordinated-care framework.
  • Conduct outreach and connect clients with GLIDE services across life domains and relevant external resources to achieve stability.
  • Develop outreach and educational materials for the broader community.
  • Assist the leadership team in coordinating special events promoting Journey Home efforts and GLIDE services.
  • Support and integrate with GLIDE's neighborhood engagement and street crisis-response programs, including the welcome center, community safety, community ambassadors, and Journey Home services.
  • Participate in self-leadership, individual supervision, team meetings, community meetings, professional development, training, and related opportunities.
  • Obtain jail clearance in San Francisco and other Bay Area counties when needed to enter city and county jails and advocate for incarcerated clients, along with other approved clearances as necessary.
Desired Qualifications
  • California Community Health Worker Certification.
