- Current Iowa Physical Therapist license or eligibility for licensure.
- Strong communication and interpersonal skills.
- Ability to work collaboratively within an interdisciplinary rehabilitation team.
- Passion for serving older adults in a Skilled Nursing Facility and post-acute rehabilitation setting.
- Evaluate patients following physician referral and establish individualized plans of care.
- Develop functional treatment goals that promote mobility and independence.
- Deliver skilled physical therapy interventions in accordance with evidence-based practice.
- Assess gait, balance, strength, endurance, transfers, and functional mobility.
- Supervise and collaborate with Physical Therapist Assistants.
- Communicate patient progress with physicians, nursing staff, families, and interdisciplinary team members.
- Participate in care conferences and discharge planning meetings.
- Educate patients and caregivers regarding safety, mobility, and home exercise programs.
- Maintain accurate and timely clinical documentation.
- Support quality outcomes and patient satisfaction initiatives.
