Requirements
  • A Master's in Social Work from a Council on Social Work Education-accredited program or a related behavioral health field.
  • A current, active license with the Board of Social Work or related licensing board in good standing within the practice state.
  • At least 12 months of recent experience, within the last 2 years, providing evidence-based individual counseling in a mental health or healthcare setting.
  • Comfort navigating electronic medical records, clinical documentation, and basic word processing and data entry, ideally with 12 months of recent experience.
Responsibilities
  • Provide individual and/or family counseling to help patients manage stress, improve relationships, and develop healthier coping strategies.
  • Connect patients with long-term therapy, psychiatric services, or outside resources when needed to ensure continuity of care.
  • Collaborate with medical providers to understand each patient's health journey and contribute to the integrated care team.
  • Raise awareness of mental health services through client-driven wellness initiatives, proactive check-ins, workshops, and educational programs.
  • Identify and address social determinants of health, such as access to food or transportation, by connecting patients with necessary community resources.
  • Build collaborative relationships with health center staff and leadership and adapt to evolving center and patient needs.

  • Previous experience providing integrated behavioral health services in a primary care setting within the last 3 years.

Clinics provide primary care, urgent care, health screenings, wellness coaching, and disease management, funded by employer subscriptions or fee-for-service so employees pay little or no co-pays. The clinics bring care directly to the workplace, focusing on preventive care and proactive management to lower medical costs and reduce downtime. The goal is to improve employee health, increase productivity, and reduce absenteeism by making affordable, convenient care available through employer-sponsored programs.
