Requirements
  • An associate degree in nursing is required.
  • A current registered nurse license is required; a compact license is preferred.
  • Care management experience is required.
  • Intermediate skills with Microsoft Office Suite products, including Outlook and Teams, are required.
  • The ability to work effectively in a primarily remote environment is required, including home internet supporting at least 25 Mbps download and 10 Mbps upload, a dedicated workspace without ongoing interruptions, and protection of member protected health information and Health Insurance Portability and Accountability Act information.
Responsibilities
  • Manage the overall care management of an assigned patient panel by applying nursing expertise and knowledge of value-based care operations.
  • Establish trusting and empathetic relationships with patients and families to provide clinical and emotional support and foster collaboration throughout their care journey.
  • Serve as an advocate and community liaison for patients to ensure timely resources and support while navigating the healthcare system and maintaining compliance with the primary care team’s or nephrologist’s treatment plan.
  • Assess panel patients’ and caregivers’ needs, including social determinants of health, based on values, care goals, and individual preferences, and translate findings into patient-centered actionable care plans.
  • Coordinate an interdisciplinary approach to continuity of care and reduction of fragmentation, including kidney disease progression management, utilization management, and provider coordination through active care plan management.
  • Monitor and evaluate care management plans regularly and modify interventions as necessary.
  • Follow evidence-based care management guidelines and established workflow protocols to deliver efficient, patient-centered care aligned with organizational goals, quality metrics, and regulatory and payer requirements.
  • Collaborate with physician partners, community providers, Advanced Practice Providers, and other clinical disciplines to create, implement, and manage integrated care plans.
  • Identify cost-effective measures that support value-based care goals, improve patient outcomes and quality, and manage resource utilization.
  • Facilitate patient and caregiver education on treatment options and empower patients to make informed care decisions.
  • Support seamless transitions of care between care settings by proactively addressing potential barriers and collaborating with interdisciplinary teams.
  • Participate in clinical huddles and patient care conferences for patients under care management as needed.
  • Participate in organizational process improvement by identifying opportunities and executing action plans to optimize care management workflows, patient engagement processes, patient care efforts, and protocols.
  • Prepare reports and other deliverables communicating program changes or developments to appropriate stakeholders.
  • Collect data and prepare and deliver reports with program leaders on program success, patient outcomes, and patient and caregiver satisfaction.
  • Perform other duties consistent with the role as assigned.
Desired Qualifications
  • A compact registered nurse license is preferred.
  • Certified Case Manager certification is preferred.
