Requirements
  • Current Registered Nurse licensure in good standing in the state or states of practice.
  • Recent hospice and home health experience, or long-term care or intensive care unit experience.
  • Ability to collaborate daily with nurses, social workers, chaplains, volunteers, and certified nursing assistant leadership.
  • Ability to work independently with phone, email, written communication, meetings, and an understanding of hospice rules and regulations.
  • Time management skills to manage visits, documentation, meetings, scheduled visits, required meetings, urgent needs, and multiple competing concerns.
  • Ability to learn an electronic medical records system.
  • Willingness to drive with reliable transportation, a valid driver's license, and auto insurance to travel across the service area.
Responsibilities
  • Provide care and services for a group of patients in their residences, including private homes, skilled nursing facilities, and assisted living facilities.
  • Contribute to the interdisciplinary team by providing expertise regarding psychosocial assessments, interventions, and community resources.
  • Work in a case management, admissions, or resource role.
  • Visit patients where they reside.
  • Prioritize and triage multiple concerns and respond to urgent needs.
