Requirements
  • A Doctor of Pharmacy doctorate or registration as a pharmacist under the stated RPH pathway is required.
  • Knowledge of pharmacokinetic principles is required for medication dosing.
  • The pharmacist must be able to monitor patients for response to medications.
  • The pharmacist must identify and document sentinel or serious events, near misses, medication errors, and adverse drug reactions in the electronic reporting system.
  • The pharmacist must promote and assure compliance with the hospital formulary and policies and procedures defined by the Pharmacy and Therapeutics Committee.
  • The pharmacist must provide care appropriate to the patient's chronological age, developmental age, social maturity, and cultural background.
  • Registration as a pharmacist in the state of Pennsylvania is required.
  • Certification is required based on the primary work location.
  • The role requires the ability to lift and carry 25 pounds, sit and stand frequently, and use a keyboard frequently.
  • Patient care providers may be required to kneel, bend, squat, and perform cardiopulmonary resuscitation.
Responsibilities
  • The Clinical Pharmacist oversees the medication distribution system for the assigned area.
  • The Clinical Pharmacist identifies, prevents, and resolves drug therapy problems by ensuring that each medication is indicated, effective, safe, and convenient for the patient.
  • The pharmacist assures the appropriateness of drug therapy and compliance with hospital policies.
  • The pharmacist oversees the distribution of medications throughout Thomas Jefferson University Hospital.
  • The pharmacist supports implementation of processes that assure the quality of pharmacy services.
  • The pharmacist trains assigned staff, such as residents, interns, technicians, and new pharmacists.
  • The pharmacist precepts student pharmacists as assigned.
Desired Qualifications
  • One year of PGY1 pharmacy residency experience is preferred.
  • Microsoft Word and Microsoft Outlook skills are preferred.
