<h3>Your future role at a glance</h3><ul> <li>Location: Elk Grove Village, IL </li> <li>Facility: Ascension Alexian Brothers Rehabilitation Hospital </li> <li>Department: Inpatient </li> <li>Schedule: Day shift | PRN </li> </ul> <h3>How you’ll make an impact in this role</h3><ul><li>Establish, revise, and evaluate an effective, patient-centered plan of care that involves the patient and/or family.</li><li>Advance clinical treatments within a variety of diagnoses including Ortho, Neuro, Oncology, ICU, Cardio, and Pulmonary.</li><li>You will collaborate with multi-disciplinary teams to establish a plan of care and discharge recommendations.</li><li>Perform therapy interventions utilizing standard physical therapy techniques and skills as appropriate for the condition of the patient.</li><li>Minimum weekend and holiday requirement.</li></ul><h3>What minimum requirements you’ll need</h3>Licensure / Certification / Registration:<ul><li>BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.</li><li>Physical Therapy (PT) license credentialed from the Illinois Department of Financial and Professional Regulation prior to hire date or job transfer date required. A valid license pending letter will be accepted until license is issued.</li></ul>Education:<ul><li>Bachelor's degree required.
