Requirements
  • A minimum Master of Social Work degree is required, with licensure preferred.
  • At least one year of experience working in child welfare with at-risk youth is required.
  • A valid New York State driver's license is required.
  • Ability to assess and engage youth and families is required.
  • Ability to travel throughout the community, secure resources and services, and adhere to the MST 120-day targeted treatment model is required.
  • Willingness to adjust the schedule as needed to ensure service needs are met is required.
Responsibilities
  • Provide aftercare support to cases transitioning to trial discharge using the MST-FIT model of treatment and therapy.
  • Provide additional support, as necessary, to cases transitioning from trial discharge MST-FIT to return to placement.
  • Provide crisis intervention and conflict resolution to youths and their families throughout the week, including evenings and weekends as required on a consistent rotation schedule.
  • Conduct virtual and in-person sessions with resident youth and families.
  • Complete weekly case, collateral, and other contact documentation within 48 hours of occurrence.
  • Ensure ongoing support and collaboration across MST and ITM programs and staff on behalf of residents.
  • Prepare and submit MST-FIT engagement court reports for scheduled permanency hearings.
  • Prepare for and attend weekly and other ITM learning community meetings and trainings.
  • Attend mandatory monthly trainings, including 7-day ITM and 5-day MST-FIT booster trainings.
  • Prepare for and attend weekly supervision and MST-FIT group consultations as scheduled.
  • Prepare for and attend monthly MST-FIT psychiatric consultation as scheduled.
  • Support the social service department with resident transition through MST-FIT treatment engagement.
  • Perform case management responsibilities for youth using the MST approach and complete associated administrative tasks in accordance with ACS and OCFS regulations.
  • Drive residents in the employee's vehicle when required for field work.
  • Perform home visits and travel to other locations as part of the hybrid/in-field schedule.
Desired Qualifications
  • Former case management experience is a plus.
  • Licensure is preferred.
