- MD or DO degree with an active, unrestricted medical license, with willingness to obtain licensure in additional target states.
- Active, unrestricted Drug Enforcement Administration registration.

- A clean compliance profile with no Office of Inspector General or System for Award Management exclusions, state board discipline, or material malpractice history.
- Experience supervising nurse practitioners or serving as a Medical Director in post-acute, skilled nursing facility, home health, telehealth, or value-based care settings.
- Comfort with friendly professional-corporation arrangements, including stock-transfer restrictions, succession agreements, and management-services-organization structures.
- Serve as the sole or majority shareholder, director, and President of one or more state-specific professional corporations.
- Provide clinical governance by approving clinical protocols, quality standards, and scope-of-practice policies for nurse-practitioner-delivered remote patient monitoring, chronic care coordination, and transitional care.
- Execute and maintain collaborative-practice or supervision agreements with nurse practitioners as required by state law.
- Oversee credentialing, peer review, and clinical-quality committees.
- Serve as, or designate, the Medical Director for applicable programs and sign off on clinical policies, standing orders, and care pathways.
- Maintain Medicare enrollment for the professional corporation and reassignment of billing rights consistent with program requirements.
- Partner with the management-services organization on compliant operations while retaining exclusive authority over clinical decisions in accordance with corporate-practice-of-medicine requirements.
- Participate in periodic governance meetings, audits, and payer or regulatory inquiries as needed.
- Board certification in Internal Medicine, Family Medicine, Geriatrics, or Physical Medicine and Rehabilitation.
- Prior experience as a professional-corporation owner for a telehealth, remote-patient-monitoring, or digital-health company.
- Multi-state licensure, especially in California, Texas, New York, New Jersey, Ohio, and other corporate-practice-of-medicine-strict states, or participation in the Interstate Medical Licensure Compact.
- Skilled-nursing-facility or post-acute Medical Director experience and familiarity with Patient-Driven Payment Model, remote-patient-monitoring or chronic-care-management billing codes 99453, 99454, 99457, and 99458, and Centers for Medicare & Medicaid Services Conditions of Participation.
- Experience with quality programs, utilization management or peer review, and payer credentialing.
