Requirements
  • A Bachelor's degree in social work, Psychology, Education, Rehabilitation, Recreation Therapy, Sociology, Nursing, or a related field approved by the Illinois Department of Human Services is required, with an alternative qualification for certain candidates of five or more years of developmental disabilities experience and a high school diploma.
  • At least 1 year of experience working with people with developmental disabilities is required.
  • At least 1 year of supervisory experience is required.
  • Company-provided CPR, first aid, Crisis Prevention Institute, direct support professional, and medication administration training must be completed within the first 4 months and annually thereafter where required.
  • An Illinois Food Service and Sanitation certificate is required within the first 6 months and must be renewed as required.
  • Passing the state-required TABE literacy assessment before employment is required.
  • Proficiency with Microsoft Word, Excel, and Outlook is required.
  • Passing criminal background, state and federal registry, DCFS Abuse and Neglect Tracking System, and driving history checks is required.
  • A valid driver's license, compliance with company driving criteria, and meeting the applicable minimum age requirement to drive company vans are required.
  • Use of a personal insured vehicle for company-related travel is required.
Responsibilities
  • Manage housing programs that provide home-based living opportunities for people with disabilities and support the highest possible level of independence.
  • Assist residents with activities of daily living, including grocery shopping, cooking, cleaning, laundry, transportation, medication documentation, bathing, and hygiene.
  • Plan, coordinate, supervise, evaluate, and document treatment services and resident care while ensuring regulatory and funding-agreement compliance.
  • Enforce safety and house rules, complete inspections, provide personal protective equipment, record accidents and illnesses, and coordinate facility and grounds maintenance.
  • Follow the budget, control expenses and inventory, submit receipts and bills, approve purchase orders and expenditures, and assist with financial reviews and public relations.
  • Promote person-centered services and engage residents in life and recreational activities that develop independent living, self-confidence, communication, and practical skills.
  • Ensure appropriate services are provided, complete required documentation, write and update Individual Habilitation Plan, Person-Centered Plan, and Individualized Adult Transition Plan documentation, and supervise resident finances as prescribed by the interdisciplinary team.
  • Participate in prospective resident screening, interviews, and admission decisions.
  • Design and implement Individual Habilitation Plans, Individual Service Plans, and Individualized Adult Transition Plans; chair the interdisciplinary team; and supervise plan reviews and revisions.
  • Chair staffing and resident monitoring processes, ensure treatment plans follow Active Treatment guidelines, and integrate facility, community, and day-program resources.
  • Support family and guardian participation and visitation and provide emergency, medical, and dental aid, evaluations, and treatment according to guidelines.
  • Maintain a clean, safe, pleasant, homelike residence and comply with agency policies, procedures, and Occupational Safety and Health Administration standards.
  • Participate in the Human Rights and Behavior Management Committee.
  • Schedule routine maintenance and minor repairs and report major repairs or facility needs to administration.
  • Assist administration with budgets, financial reports, capital expenditure requests, and preparation for onsite visits and surveys.
  • Lead and supervise 8–12 staff members; hire, train, empower, mentor, develop, schedule, and evaluate team members; communicate job expectations; and conduct performance evaluation and problem resolution.
  • Plan, organize, implement, direct, and evaluate assigned site activities and performance and enforce systems, policies, and procedures.
