- An active, unrestricted Licensed Practical Nurse license in Massachusetts with no board action.
- Experience in a clinical or medical setting is preferred.
- Experience working in a multidisciplinary care model across network providers, health plan care management support, and employed clinicians.
- Ability to multitask and work under pressure while maintaining a positive member experience.
- Ability to recognize when to escalate and to act professionally in a potential emergency.
- Technical skills to navigate documentation systems and tools, and ability to learn new systems as the company grows.
- Strong written skills.
- Ability to drive during daytime, nighttime, or inclement weather.
- A valid driver's license with a safe driving record.
- State minimum automobile insurance coverage.
- Comfort with change and ambiguity as the business, strategy, organization, and priorities evolve.
- Administer clinical assessments and screening tools at different points throughout the care timeline, with support from nurse practitioners and registered nurses.
- Prioritize member needs based on assessment results and assign tasks to the interdisciplinary care team.
- Be accountable for care plan development within the multidisciplinary care team.
- Participate in internal and joint rounds to support an optimal and coordinated member experience.
- Learn and use Tuesday Health's electronic systems, tools, and technology to deliver coordinated care.
- Build trusting relationships with members, using listening and empathy in interactions.
- Serve as the quarterback of the multidisciplinary team transforming care for members with serious illness.
