  • An active, unrestricted Minnesota Registered Nurse license required, or ability to obtain by start date. Multi-state compact license preferred
  • Willingness to obtain additional cross-state licensure as needed
  • Minimum of 2 years’ experience as an Registered Nurse in a clinical setting
  • Prior experience working as a triage Registered Nurse or Registered Nurse case manager required
  • Exceptional communication skills and patient-centered focus
  • Ability to monitor and evaluate opportunities for cost-effective care with high-quality outcomes
  • Demonstrate effective, independent nursing judgement and skills
  • Demonstrate skill and experience in effectively collaborating with care team members
  • Demonstrate skill in effective use of electronic health records
  • Demonstrate flexibility, organization, and appropriate decision-making in ambiguous situations
Responsibilities
  • Transitional Care Management: Focused on outreach during the 30 days after discharge with the goal of reducing emergency room visits, hospitalizations, readmissions, and post-acute stays
  • Transitional Care Management: Reviews daily Admission, Discharge, and Transfer feeds and initiates contact with patients within 2 days of discharge
  • Transitional Care Management: Schedule follow-up for Transitional Care Management visit with the Primary Care Physician within 7 days of discharge
  • Transitional Care Management: Complete weekly outreach for 30-day post-discharge
  • Transitional Care Management: Review discharge plan and ensures services (e.g. Physical Therapy, Home Health) are in place; provides community resources for Social Determinants of Health needs
  • Transitional Care Management: Provides patient/ family education regarding health conditions and disease self-management
  • Transitional Care Management: Completes medication reconciliation and monitors adherence
  • Transitional Care Management: Documents all telephone/ video interactions in EHR patient chart
  • Transitional Care Management: Identifies patterns and episodes of care that are predictive of future needs and services
  • Transitional Care Management: Coordinates care with other providers involved in the patient’s care as needed
  • Triage: Respond to patient telephone calls, refill requests, and electronic health messages
  • Triage: Demonstrates critical thinking skills and expert nursing judgement to provide appropriate recommendations for care utilizing an approved evidence-based telephone nursing triage protocol
  • Triage: Works with the RN scope of practice and standing orders to provide prescription refills, medication titration, and home treatments
  • Triage: Educates patient regarding test results and medical conditions, and explains medical terms and procedures
  • Triage: Maintains a professional demeanor and focus on patient-centered care
Desired Qualifications
  • Prior experience working in telephonic case management preferred
  • Knowledge of Medicare and NCQA regulatory transitions of care preferred
  • Experience in value-based care preferred

Kristen Helton is no stranger to the healthcare needs of patients. After co-founding biosensor company Profusa, she went on to work at Amazon, where she helped found and lead the company’s Amazon Care division in 2019.During the next three years, there was a focus on creating a positive experience for patients. And though the company deployed a feedback loop in order to make experiences even better, Helton found that patients would still say they weren’t satisfied.When Amazon Care shut down in 2022, Helton decided to find a way to address that dissatisfaction, focusing on a demographic where she felt care was lacking: care for women aged 65 and older.She founded Herself Health last year, touting it as “the first healthcare company” built just for this population of patients.“Women are not feeling heard, and they’re not well served,” Helton told TechCrunch. “It takes a long time to get an appointment with their doctor, and women are feeling rushed to tell them everything in 10 minutes.
