Requirements
  • An active Registered Nurse license in the state of New Mexico is required.
  • At least 1 year of clinical nursing experience is required.
  • Proficiency in OASIS documentation and home health regulations is required.
  • The role requires strong clinical judgment and attention to detail.
  • The role requires the ability to work independently with reliable transportation.
  • Current CPR certification is required.
  • A valid driver’s license and auto insurance are required.
Responsibilities
  • Perform initial and ongoing assessments to develop and update personalized care plans.
  • Deliver skilled nursing services and specialized procedures according to physician orders.
  • Complete timely and accurate documentation, including OASIS assessments.
  • Educate patients and families on safety, medications, and health conditions.
  • Communicate with physicians regarding changes in patient condition.
  • Supervise and support LPNs and HHAs, including conducting supervisory visits.
  • Coordinate care with the interdisciplinary team and participate in team conferences.
  • Ensure compliance with clinical, regulatory, and documentation standards.
Desired Qualifications
  • Home health nursing experience is preferred.
