Requirements
  • PharmD required or equivalent
  • Texas Pharmacy license required at time of hire
  • Knowledge of and compliance with state and federal pharmacy laws and regulations
Responsibilities
  • Demonstrating proficiency in all pharmacist functions, such as interpreting, transcribing, or verifying medication orders
  • Attend and actively participate in daily multidisciplinary and bedside rounds
  • Complete drug utilization reviews to promote rational drug therapy
  • Lead and develop clinical services
  • Mentor and educate staff to ensure accurate and appropriate documentation of clinical activities as needed
  • Participating and leading pharmacy clinical/operational programs, monitoring patient medication regimens and providing appropriate interventions
  • Review, interpret, and input treatment orders to ensure accuracy of dosage, treatment schedule, route of administration, and safe administration based on laboratory information and evidence based practices
  • Provide and support clinical pharmacy responsibilities by collaborating with other hospital disciplines to support service lines
  • Lead and develop pharmacy clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and other programs as defined by Pharmacy Leadership
  • Provide effective medication and practice-related education and counseling to patients, caregivers, and medical staff
  • Collaborate with medical and nursing staff to optimize the pharmacotherapeutic aspects of patient care including infusion and outpatient medication management
  • Provide information to staff members and/or patients on medication uses, drug interactions, and potential side effects
  • Oversees the preparation of treatments, preparation of investigational treatments, prepackaging of medications, and drug returns
  • Maintains records on patient prescriptions, charges, and inventory
  • Ensure pharmacy technician duties are performed completely, safely and without risk of harm to patients
  • Analyzes, compares, and evaluates various courses of action; makes independent decisions
  • Maintains responsibilities relating to pharmacy requirements for FACT-JACIE accreditation
  • Management of stem cell transplant and cellular therapy therapies, order sets, and REMS requirements
  • Provide staff education regarding cellular therapy management Provide direct clinical pharmacy support for the stem cell transplant and cellular therapy patients
  • Aid in the precepting of pharmacy students and residents
  • Performs all other duties as assigned
