Requirements
  • An active, unrestricted state nursing license is required; compact licensure is preferred.
  • At least 3 years of experience as a Registered Nurse in a direct, patient-facing care delivery role is required.
  • Current CPR certification is required.
  • Prior telehealth experience is preferred, including experience using a telehealth platform.
  • Home care experience is preferred.
  • Previous experience in hospice, palliative care, geriatrics, or other complex patient healthcare is strongly preferred.
  • A valid driver's license with an acceptable driving record is required.
  • Proof of an active auto insurance policy is required.
  • Proficiency with Microsoft Office, including Excel, Word, and G Suite, is required.
  • Proficiency with Microsoft 365, electronic health records, scheduling software, and RingCentral applications is required.
  • The ability to work independently and make sound decisions while following established protocols and guidelines is required.
  • Organizational and time-management skills are required to handle administrative tasks and facilitate telehealth consultations.
  • Experience working with culturally diverse and underserved populations is required.
  • The ability to lift up to 20 pounds, assist with patient lifting or transfers involving up to 50 pounds, and assist with weights exceeding 100 pounds is required.
  • The ability to stand for extended periods, drive to patient and community-resource locations, and perform the required fine-motor and visual tasks is required.
Responsibilities
  • Deliver urgent, in-home patient care under the direction of a licensed provider throughout the assigned market, including initial assessments, routine follow-up visits, and ongoing care coordination.
  • Conduct comprehensive home-safety evaluations and document findings during patient visits.
  • Obtain patient histories, review medications and treatment plans, perform physical assessments, and identify and address health concerns observed or reported during home visits.
  • Perform clinical interventions as appropriate and at the direction of the overseeing provider, including oral, sublingual, intramuscular, and intravenous medication administration; wound care; point-of-care testing; phlebotomy; Foley catheter care; intravenous fluid administration; and other approved nursing procedures.
  • Maintain accurate, timely, and complete patient records, updating documentation at each patient encounter.
  • Provide patient and caregiver education as directed by the nurse practitioner and interdisciplinary care team.
  • Deliver patient-centered care while meeting established clinical, quality, and operational performance standards.
  • Support care transitions, end-of-life discussions, palliative care interventions, and the closure of preventive-care and Healthcare Effectiveness Data and Information Set quality gaps, including screenings and medication-adherence initiatives.
  • Complete and submit required documentation, reports, and administrative paperwork within established timelines.
  • Participate in interdisciplinary team meetings and provide updates from patient and family interactions to support care planning.
  • Collaborate with advanced-practice providers, nurses, social workers, behavioral-health specialists, and other care-team members to develop and implement individualized patient care plans.
  • Attend required training programs covering medication administration, clinical-equipment use, new protocols, and ongoing professional development.
  • Build and maintain relationships with local healthcare providers, community organizations, caregivers, and referral sources.
  • Use internal and external resources, including behavioral-health services and community-based programs, to address patient needs.
  • Maintain all required active, unrestricted state licensure and certifications.
  • Follow professional standards of practice, ethical conduct, patient-confidentiality requirements, and organizational policies.
  • Work the tentative scheduled hours of 10:00 a.m. to 7:00 p.m. Eastern Time.
  • Travel locally, up to 100% of the time, to deliver patient care.
