Requirements

  • Current Illinois licensure or registration, or eligibility for it, is required.

Responsibilities
  • Evaluate and treat geriatric patients in the inpatient setting to improve outcomes, transitions of care to post-acute settings, geriatric syndromes, discharge planning, and documentation affecting the O-E Mortality model.
  • Provide independent interventions and evaluation of care.
  • Conduct daily rounds from Monday through Friday, assess hospitalized medical patients, and plan discharges.
  • Work with social workers and case managers on hospital floors to identify appropriate discharge candidates early in admissions and prepare them for discharge.
  • Participate in developing, reviewing, and evaluating standards and protocols.
  • Communicate regularly with physicians, nurses, and skilled nursing facility staff to ensure pertinent patient information is received by the team.
  • Assess newly transferred patients and create treatment plans.
  • Take patient histories, conduct physical examinations, and write progress notes.
  • Prepare patient workups and summaries that note pertinent positive and negative findings.
  • Design patient education programs containing information needed for informed health care and treatment decisions.
  • Evaluate goals and outcomes based on benefit versus burden, safety, quality, and cost-effectiveness for the patient, family, and organization.
  • Perform other related work as needed.
