Requirements
  • A Bachelor’s degree in Social Work (BSW) is required.
  • Comply with Illinois and/or Michigan state licensing and certification requirements in the state or jurisdiction where practice occurs.
  • A Social Worker with a BSW who has not completed three years of supervised clinical hours must complete the clinical practicum required to sit for the examination and apply for social-work licensure.
  • All BSW mission partners must obtain the applicable license from the Department of Professional Regulation in their designated state of practice within four years of hire, subject to the stated timeline and credit for documented supervised clinical hours.
  • Identify a clinical supervisor by day 91 of hire and complete the supervision plan within three years.
  • Submit the state application within one month after completing the three-year clinical supervision requirement.
  • A Social Worker with an MSW who is not licensed must obtain the applicable state license within one year of hire.
  • Begin the MSW licensing application process by day 91 of hire.
  • Three years of experience caring for patients is required.
  • Demonstrated broad knowledge of chronic illness and medical and community resources.
  • Working knowledge of a clinical electronic medical record system and Microsoft Office applications.
  • Ability to work a flexible schedule including weekends, holidays, and on-call rotations.
  • Ability to use independent judgment, make decisions, and work with little supervision.
  • Substantial interpersonal skills to develop and maintain cooperative working relationships with personnel from other departments.
  • Excellent time-management skills.
  • Ability to communicate effectively with patients and families across varied health issues, age groups, and socioeconomic backgrounds.
  • A positive and empathetic approach is required.
Responsibilities
  • Assess, plan, implement, coordinate, and monitor interventions to promote optimal clinical and resource outcomes.
  • Develop comprehensive plans of care for patients.
  • Communicate on an ongoing basis and collaborate with the Interdisciplinary Care Team to promote quality care.
  • Coordinate health care services on behalf of patients so care is delivered at the appropriate level and clinical outcomes and satisfaction are optimized.
  • Assess and intervene on complex psychosocial and behavioral-health issues associated with population management and transition plans.
  • Provide services using the knowledge and skills necessary for the assigned patient population.
  • Work with the Care Management Division leadership team to align work with strategic goals and OSF Key Results.

  • Experience with chronic, complex illnesses and adult and/or pediatric care management.

410 Medical raises $12M to scale emergency care device LifeFlow

410 Medical has secured $12 million in financing to expand its commercial operations and develop its next-generation LifeFlow infuser. Orlando Health Ventures led the oversubscribed round, with participation from Hatteras Venture Partners, Ballad Health, OSF Healthcare, Rex Health Ventures, and several other healthcare investors. The Durham, North Carolina-based medical device company's LifeFlow products are currently used by more than 500 hospitals and EMS agencies across the US to deliver fluids and blood quickly to patients experiencing sepsis, hemorrhagic shock, and other life-threatening conditions. Nearly 80,000 patients have been treated using LifeFlow technology. The funding will support commercial expansion and the launch of an enhanced LifeFlow infuser, planned for 2027. Founded in 2013, 410 Medical received FDA clearance for LifeFlow in 2016.
