Requirements
  • A bachelor's degree and two years of professional experience in human services or social services programs.
  • Ability to work with adolescents and families affected by substance use and co-occurring behavioral health concerns.
  • Ability to engage adolescents and families who may have difficulty accessing or remaining engaged in treatment services.
  • Familiarity with local community resources.
  • Public speaking and/or classroom management ability.
  • Ability to work independently while functioning collaboratively within an interdisciplinary team.
  • Ability to deal effectively with stressful situations.
  • Ability to learn and use new processes, tools, and equipment as required.
  • Computer literacy.
  • Ability to work overtime as required.
  • Ability to work rotating shifts.
  • Ability to perform the listed physical duties, including lifting and carrying up to 35 pounds unassisted and assisting with patient movement as specified.
  • Ability to meet the listed vision, hearing, dexterity, mobility, and respirator-use qualifications.
Responsibilities
  • Coordinate treatment linkages and referrals for adolescents and caregivers referred to the Family Tree Program, including screening referrals, conducting phone screens, gathering information from referral organizations, and presenting cases at weekly team meetings for clinician assignment.
  • Develop comprehensive recovery plans and implement wraparound services for adolescents and family members, including case management, community navigation, referrals to medical or mental health providers, needs and barriers assessments, transportation coordination, and documentation from referral through discharge.
  • Implement Screening, Brief Intervention and Referral to Treatment and CATCH My Breath programs for youth connected to partner schools and organizations.
  • Assist with community-based awareness events and education, and provide initial and booster training in these models as needed.
  • Abide by federal and state confidentiality and privacy requirements, Medical University of South Carolina policies and procedures, and grant requirements and objectives.
  • Ensure the project team is knowledgeable about issues affecting the functioning and management of Family Tree and EMPOWERR.
  • Assist with SAMHSA-required data collection involving youth and caregivers.
  • Attend weekly staff meetings and other meetings with the program coordinator and team members.
Desired Qualifications
  • A master's degree in a relevant field such as social work, public health, education, psychology, or related social sciences.
  • Interest in community outreach and building relationships with schools and other youth-serving organizations.
  • Excellent listening, verbal, written communication, and problem-solving skills.
  • Strong interpersonal skills and ability to develop rapport with adolescents, caregivers, and community partners.
  • Experience entering data into REDCap databases and knowledge of basic database design.
