Requirements
  • For Florida, hold a bachelor's degree in a human services discipline, including Social Work, Psychology, Sociology, Counseling, or a related discipline.
  • Have experience working with children and families in a case management or community resource capacity.
  • For Mississippi, hold a master's degree in a human services, behavioral health, social work, counseling, or related field.
  • For Mississippi, meet payer-required credentialing standards, which may include Licensed Social Worker, a Department of Mental Health-approved Mental Health Therapist credential, or Registered Nurse.
  • For Mississippi, have two years of experience in mental health care.
  • Meet all applicable state regulatory requirements.
  • Complete a comprehensive background investigation before joining the organization.
Responsibilities
  • Provide best-practice, quality case management services.
  • Ensure all documentation is completed according to payer, state, and Council on Accreditation standards and requirements.
  • Assist in marketing new referrals.
  • Communicate with customers, colleagues, and other individuals to answer questions, disseminate or explain information, and address complaints or concerns.
  • Develop service plans with input from the treatment team and Clinical Supervisor.
  • Connect families with available community resources and follow up with clients and agencies to document referral use and success.
  • Complete documentation within established time frames.
  • Meet with clients weekly or bi-weekly for up to 12 months.
  • Attend and participate in bi-weekly treatment team meetings.
  • Serve as a liaison with other professionals and agencies related to the client.
  • Actively recruit new referrals to keep the caseload full.
  • Provide 24/7 on-call support for clients as required by the program.
  • Ensure compliance with all state regulatory bodies.
