Requirements
  • A Bachelor's Degree.
  • Five years in a leadership position in the post-acute sector overseeing patient care programs.
  • Working knowledge of an electronic medical record system with a minimum of five years in the area of specialty.
  • Proficiency in Microsoft Office, Word, PowerPoint, and Excel.
  • Ability to lift and carry 25 pounds, with frequent sitting, standing, and keyboard use.
  • Patient care providers may be required to perform role-specific activities including kneeling, bending, squatting, and cardiopulmonary resuscitation.
Responsibilities
  • Manage daily operations of the program and support staff in facilitating discharges, lightening rounds, team conferences, and family meetings where needed.
  • Manage program compliance with applicable regulatory agencies, including The Joint Commission, the Commission on Accreditation of Rehabilitation Facilities, and the Centers for Medicare & Medicaid Services.
  • Use resources to plan and predict rehabilitation patient outcomes and quality indicators.
  • Support teamwork and interdisciplinary care and provide onsite support to program staff.
  • Coordinate and lead programmatic meetings with the Program Director and establish systematic communication with program team members.
  • Implement programmatic operational and business initiatives per the Program Director.
  • Participate in the daily admissions call and daily safety huddle.
  • Be responsible for local outcomes such as discharge to community, safety, and patient experience; present at ROQUE and support team members' understanding and presentation of data.
  • Manage appropriate length of stay while maintaining high-quality care for patients in the program.
  • Tour families and caregivers of potential patients.
  • Provide support to the Program Director as needed.
  • Plan and oversee community activities in collaboration with the Program Director, including advocacy groups, education, and marketing.
  • Establish mutually beneficial relationships with program counterparts at other sites.
  • Perform other duties as assigned, including enterprise falls committee and enterprise initiatives pertaining to program areas.
Desired Qualifications
  • A Master's Degree, Master of Business Administration, or Master of Health Administration is preferred.
