Requirements
  • Graduation from an accredited program for Licensed Practical Nurse or Licensed Vocational Nurse and current registration in the state of the community.
  • Valid continuing education credits as required by the state of the community.
  • Training in Alzheimer’s and dementia care with a focus on Validation Therapy.
  • At least one year of nursing experience in a healthcare facility.
  • Experience in emergency care and first aid.
  • Ability to communicate effectively with management, associates, residents, family members, and outside contacts.
  • Ability to make independent decisions.
  • Ability to seek out new methods and incorporate them into existing practices when applicable.
  • Willingness to disclose medication that might impair the ability to perform the job safely or competently.
  • Ability to work in a safe and alert manner.
  • Ability to take ownership of associate and resident safety.
  • Passion for working with and around senior citizens.
Responsibilities
  • Provide direct and individual nursing care to assigned residents according to nursing standards and under the supervision of the Director of Resident Care.
  • Maintain compliance with state and federal regulations and HRA standards.
  • Ensure the quality and safe delivery of health services to residents.
  • Implement the plan of care formulated by the physician and Director of Resident Care.
  • Document care accurately and promptly.
  • Assess residents and provide residents and family or caregivers with information pertinent to the diagnosis or plan of care.
  • Administer medication according to physician orders, document it on the medication administration record, observe for adverse reactions, and notify the physician as appropriate.
  • Assemble resident charts and records and verify that all necessary forms are complete and accurate.
  • Prepare incident and accident reports as needed, notify the physician and family after incidents or accidents according to policy, and document appropriately.
  • Maintain a safe and healthy working environment using safe methods and procedures consistent with clinical standards.
  • Assist residents in learning appropriate self-care activities.
  • Organize and monitor the associates and material resources needed to meet the needs of assigned residents.
  • Recognize emergencies and life-threatening situations and initiate appropriate actions.
  • Maintain confidentiality and resident rights concerning personal information.
  • Lead by example through servant leadership and the organization’s CORE Values.
  • Attend required community meetings and participate in Life Enrichment activities when able.
  • Complete required courses in accordance with HRA University.
  • Perform the required duties on a regular and predictable basis.
  • Learn how to report a missing person using the Safe Return program or equivalent.
  • Help maintain a safe and secure environment for all community residents.
  • Perform other duties requested by the supervisor.
Desired Qualifications
  • Experience in a long-term care community and/or geriatric nursing facility.
  • Experience with Microsoft Office and Outlook software.
