Requirements
  • A High School Diploma or Associate's degree may be considered in conjunction with increased years of relevant experience.
  • Specific experience in mental health services and knowledge of community resources are required.
  • A flexible work schedule must be maintained according to individual client needs, including late afternoon and evening hours as needed.
  • Access during work hours to reliable private transportation is required.
  • A valid New York State Driver License that meets The Salvation Army's insurance requirements is required.
  • Successful completion of background and drug screens is required.
Responsibilities
  • Conduct intakes for clients referred to the program.
  • Provide support, advocacy, and crisis intervention to individuals and families.
  • Devise and implement individualized service plans and intervention strategies targeting all aspects of the individual and family system while utilizing client input.
  • Identify needs not met by the program and advocate on clients' behalf, connecting clients to community and mainstream resources as appropriate.
  • Coordinate services with other service providers to provide housing assistance in a timely and efficient manner.
  • Assist clients in accessing mainstream resources and increasing income to stabilize their housing.
  • Build appropriate budgets to minimize the risk of homelessness, including transporting clients and families as needed.
  • Provide life skills training through individual and group sessions to assist clients with self-sufficiency.
  • Conduct housing inspections to ensure safe, affordable housing is obtained for clients in the program.
  • Verify tenancy with landlords initially and on an ongoing basis as needed.
  • Conduct regular home visits.
  • Ensure client service information and financial paperwork are accurately completed and recorded in a timely manner.
  • Enter all client information into HMIS and conduct quarterly reviews of client progress.
  • Participate in case conferences, staff meetings, service coordination meetings, supervision, professional training, and in-service training sessions.
Desired Qualifications
  • A Bachelor's degree in human services or a related field is preferred.
