Requirements
  • Experience with case management.
  • Ability to utilize motivational interviewing and harm reduction approaches to promote follow-through with preventive care and treatment for substance use, mental health symptoms, and high-risk behavior.
  • Ability to assess youth cases through biopsychosocial evaluations and assessments to determine case management needs and goals related to mental health support or high-risk behavior, and provide referrals to appropriate treatment or services.
  • Knowledge of best-practice service models, including trauma-informed care, harm reduction, and social justice positive youth development.
  • Knowledge of transitional aged youth experiencing housing instability is desirable for success in the role.
  • Commitment to a culturally responsive trauma-informed approach and client-centered advocacy.
  • Strong knowledge of the communities served and ability to centralize this cultural understanding in the work.
  • Commitment to justice, equity, diversity, and inclusion.
  • Ability to evaluate the completeness, accuracy, and relevance of data to determine information value and improve client services.
Responsibilities
  • Manage a caseload of up to 15 youth through housing-focused case management centered on increasing skills, removing barriers, and obtaining education plans, employment, and stable housing.
  • Create comprehensive individual case plans tailored to youths’ needs and goals related to education, employment, wellbeing, and stable housing.
  • Provide intensive case management by identifying and removing existing or potential barriers and linking youth to relevant community resources.
  • Establish rapport to increase program engagement and provide services and support.
  • Develop transition plans to guide and support youth into stable housing.
  • Coordinate weekly meetings to provide coaching and guidance on case plan goals related to housing, employment, education, wellness, and life skills.
  • Evaluate ongoing information and assessments to ensure youth safety, wellbeing, progress toward case plan goals, and the condition of their living environment.
  • Coordinate and facilitate apartment and program readiness tasks for youth move-ins and exits.
  • Lead care coordination meetings with youth and their care team regarding progress and needs, and directly address program violations or concerns.
  • Monitor site safety and cleanliness, including conducting thorough unit inspections.
  • De-escalate conflicts involving community violence, domestic violence, or suicidal ideation.
  • Provide crisis intervention and restorative justice techniques, complete safety measures such as safety plans and incident reports, and support youth through incidents.
  • Coordinate and facilitate monthly groups or workshops that develop life skills for independent living and responsible adulthood.
  • Attend community-building activities, life-skills workshops, and youth events.
  • Encourage youth participation in workshops and events to build community.
  • Meet due dates for data entry and documentation requirements.
  • Document service efforts in Apricot and the Homeless Management Information System within required timeframes.
  • Track youth progress toward goals and required outcomes.
  • Analyze outcome data to improve service delivery.
  • Attend agency-required trainings and expand knowledge of services relevant to the population.
  • Submit required agency documentation and procedures accurately and on time, including mileage, payment requests, and reimbursement documents.
  • Uphold and implement the organization’s values, including program evaluation, empowering partnerships, an inclusive learning environment, open communication, and collaboration.
  • Support program evaluation and data-quality efforts to improve impact and client services.
