- An active Texas physical therapist license in good standing is required.
- Strong clinical judgment and the ability to work independently in the field are required.

- Competency in core home health physical therapy scope is required, including transfers, gait training, balance and fall prevention, assistive device recommendations, strengthening, functional mobility, home safety assessment and modification education, and patient and caregiver coaching with practical home programs.
- Own the caseload as a case manager and remain accountable for each patient's therapy plan, progression, and outcomes.
- Complete comprehensive in-home assessments and deliver skilled physical therapy visits.
- Build and execute plans of care covering gait and balance, transfers, strengthening, functional mobility, home safety, and fall prevention.
- Set measurable goals, track objective progress, and adjust treatment when patients stall.
- Coordinate with the multidisciplinary care team by communicating changes in condition and aligning on orders.
- Manage treatment and outcomes directly, including supervising physical therapist assistants where appropriate.
- Coach patients and caregivers on practical home programs.
- Document promptly and thoroughly.
- Prior home health experience.
- Experience with electronic medical records.
- Deeper clinical experience with high-signal physical therapist populations.
- Experience supervising or mentoring physical therapist assistants and collaborating on care plans.
- Strong organization and time management.
