Requirements
  • Current Indiana licensure in nursing as a Licensed Practical Nurse or Registered Nurse, or in an allied health profession as a Physical Therapist, Occupational Therapist, or Speech-Language Pathologist.
  • Experience as a clinical liaison in acute rehabilitation or a closely related field is required.
Responsibilities
  • Build trust with patients, families, physicians, and referral sources.
  • Proactively seek referrals from hospitals, assisted living centers, home health providers, physicians, and case managers.
  • Communicate daily with the Acute Rehabilitation Unit Medical Director and Director of Rehabilitation regarding referrals, admissions, and discharges, and enter required information in UDS.
  • Develop and maintain positive relationships with hospital employees and Signet team members.
  • Present rehabilitation education and unit criteria to case managers, physicians, and other referring entities through the proper channels.
  • Provide ongoing, follow-up, and discharge communication to referral sources, patients, families, nursing staff, the medical director, and therapists.
  • Adhere to hospital and Signet Health policies and procedures, ethical and corporate compliance standards, and the professional code of conduct.
