Responsibilities
  • Translate medical complexity into plain language, coordinate care across providers, and explain insurance paperwork.
  • Build and own individualized care plans with Careway physicians addressing medical needs and social determinants, including transportation, food, home support, and digital access.
  • Identify patient barriers such as denied claims, missed referrals, lack of a primary care provider, or unaffordable medication, and persist until the issue progresses.
  • Lead difficult conversations with empathy and firm boundaries to help frustrated or overwhelmed patients move forward.
  • Coach patients on health literacy and self-management, including diabetes, chronic pain, mobility, and medication adherence.
  • Share frontline observations to improve Careway's advocacy model and tools.
Desired Qualifications
  • Background in community health or health coaching.
  • Experience with Medicare populations.
  • Fluency in Spanish or other languages.
  • Candidates with 30 or more hours of weekly availability are highly prioritized.
